00(depending on experience) Summary: Under supervision of the Director of Residential Services The Licensed Vocational/ Practical Nurse will ensure the compliance with policies and procedures of State and Federal regulations on a daily basis, oversee program implementation, record keeping, and health care.

will administer and monitor medication administration.
Will supervise and assist in the delivery of hygiene and healthcare.
Will coordinate nursing service with other consumer care services provided.
Will assure daily, implementation of Active Treatment including such areas as self-help, independent living, personal development, social skills, communication skills, leisure/recreational skills and behavior modification.
Shall review the drug regimen of each consumer weekly.
The LVN will be responsible for scheduling, communicating, and accompanying consumers to all scheduled health care appointments or emergencies locally or in San Diego and assure the necessary transportation is available.
Must understand and promote Person Centered Planning (PCP) and Focus System concepts for consumers based.
Ensure home is staffed adequately and work is performed according to job description.
Essential Duties and Responsibilities include the following.

Additional documentation is required for basic nursing and medical care delivered to consumers.

Will coordinate nursing service with other consumer care services provided such as, physician, dietary, occupational and physical therapists, consumer activities and social services.

will implement programs for consumers as planned by the Interdisciplinary Team and written ISP.
Will assure daily, implementation of Active treatment including such areas as self-help, independent living, personal development, social skills, communication skills, leisure/recreational skills and behavior modification.
Will assess and review data collection for trend and patterns.
Shall review the drug regimen of each consumer weekly, and shall document the review by initialing documents.
Will review and sign the pharmacists drug report.
Will participate as a member of the Interdisciplinary Team and communicate with family members, co-workers, and interested person within the realm consistent with rights and privacy about the ongoing status of consumers.

will be charge of the physical appearance, cleanliness and safety of each home according to the standards set by the Department of Health Services, Medi-Cal, State Fire Marshal, and ARC Policies and Procedures.
Understands and stays current with the needs and changing trends that will affect the individuals service by ARC.
Must understand and promote Person Centered Planning (PCP) and Focus System concepts for consumers based on positive reinforcement and the normalization principal.
Ensure home is staffed adequately and work is performed according to job description.
Evaluates and discipline under guidance of the Program Director and in accordance with approved personnel policies and procedures.
Promotes a better, more cooperative working atmosphere by maintaining a safe, hazard free work environment by following established standards for desirable conduct, including but not limited to discrimination, sexual harassment, and alcohol/substance abuse.
Is knowledgeable and adheres to CAL OSHA and ADA requirements as it applies to the ICF/DD-H program.

may perform other duties as assigned, including work in other functional areas to cover absences or relief, to equalize peak work periods or otherwise to balance the workload.
Will participate in Residential Team meetings, Safety Committee Meetings, Infection Control Meetings, and Human Rights Committee meetings.
Supervisory Responsibilities Directly supervises 6 to 7 employees in the Group Home.
Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.
Responsibilities include interviewing, hiring, and training and in-servicing employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
Must be flexible and accessible via pager.
