_*We are looking for a compassionate Home Health Registered Nurse (RN) or licensed vocational nurse (LVN) in Lancaster and other areas.

We are both a home health and hospice care agency seeking to hire *RNs*, *LVNs*, who are experienced, motivated, and capable of working independently at the patients’ homes to do either home health or hospice care.
Multi-lingual clinician's speakers are preferred, but not required.

*Duties:*
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living.
- Perform specialized procedures such as treatments, exams and dementia care.
- Ensure accurate documentation of patient information and maintain up-to-date medical records.
- Collaborate with healthcare team members to develop and implement individualized care plans.
- Educate patients and their families on disease management, treatment options, and preventive measures.
- Adhere to all relevant healthcare regulations and maintain a safe and clean work environment.
*Requirements:*
- Valid license as a Vocational Nurse/ Registered Nurse in the state of California.
- Proven experience in providing patient care across various medical specialties.
- Knowledge of documentation requirements.
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
- Ability to prioritize tasks and work efficiently.
- Compassionate and empathetic approach to patient care.
- Reliable method of transportation
- Eligible for Employment in the United States
- Good command of the English language, both speaking and writing
