Job Type: PerDiem/Par-Time Potential Full Time HOSPICE SOCIAL WORKER (MSW) Aria Hospice is the only hospice in Imperial County that is recognized by the Veteran’s Affairs Office as a Service-Disabled Veteran-Owned Small Business (SDVOSB).
Aria Hospice is owned and operated by a local veteran who is also a retired Medicare Surveyor from the California Department of Public Health.
Aria Hospice is a participant of the We Honor Veterans and No Veteran Dies Alone Programs.
Aria Hospice for is not a hospice that belongs to a national chain, therefore, we provide the best patient-family centric services with the highest rates of patient visits in Imperial County, in addition, our nurses have the smallest case load in the industry in order to provide the best and compassionate hospice care.
JOB SUMMARY The hospice social worker is responsible for the implementation of standards of care for hospice patients and their families.
Duties and Responsibilities - The following are basic duties and responsibilities: * Assesses the psychological status of patient's condition related to his illness and coordinates care with the Interdisciplinary Group (IDG).
* Plan intervention based on evaluation and findings.
Develops care plans.
* Maintains all documentation of visits and contacts with patients via electronic medical records.
* Provides information to patients and families/caregivers regarding family/caregivers services and serves as a liaison between these groups.
* Participates in the IDG meetings and participates in the development of the patient's plan of care and case conferences as required.
* Participates in discharge planning * Takes other duties as delegated by the Director of Patient Care Services/Nursing Supervisor.

JOB REQUIREMENTS * Graduate of a MSW program at a school accredited by the Council on Social Work Education.
* Minimum of one year experience in health care, preferable in home health or hospice care.
* Excellent verbal and written communication, and organizational skills.
* Current CPR certification.
* Must be a licensed driver with an automobile that is insured following state requirements.
* Spanish speaking preferred, not required.
Job Types: Full-time, Part-time Pay: $100.
